- •Liaise with providers and patients to gather detailed clinical information
- •Escalate concerns about suboptimal or harmful care to the Near Patient Physician Lead
- •Assist the Near Patient Medical Collection Team in obtaining discharge summaries and hospital records
- •Ensure medical information aligns with admission details and update records accordingly
- •Offer on-site patient support services, including language support and chaperone services
- •Establish and maintain relationships with providers for clinical quality assurance
