Requirements
- U.S. Citizenship
- Secret or Top Secret Clearance
- Skill in understanding diagnoses to assist patients
- Full, active, unrestricted RN or LCSW license
- Two years nursing or case management experience
- Certified case manager or obtain certification within first year
- Knowledge of patient education principles and management
- Ability to seek feedback for knowledge expansion
- Consult with healthcare providers and negotiate services
- Maintain clinical practice privileges
- Maintain credentialing requirements
- Fluent English oral and written communication skills
- Team coordination, facilitation, and communication skills
- Knowledge of laws, ethics, and practice standards
- Maintain current BCLS certification
- Previous military background/experience
Tasks
- Apply health nursing principles to analyze care problems
- Provide cost-effective care to beneficiaries
- Collect and record health assessment data
- Develop time-sensitive treatment plans
- Assess patients via telephone using protocols
- Establish patient care priorities
- Monitor and evaluate patient progress
- Oversee discharge planning activities
- Provide assistance to identify patient referral needs
- Negotiate, write, and administer service agreements
- Recognize and report quality and risk management issues
- Ensure evidence-based care using VA/DOD guidelines
- Apply knowledge of healthcare financial environment
- Critically evaluate practices using research tools
- Manage information systems for health and practice improvement
- Conduct data collection and analysis using software
- Design and present outcome analysis information
- Gauge pharmaceutical effects on patient recovery
- Assist patients to optimize function and self-care
- Enhance patient adherence to treatment plans
- Train peers and staff on patient education
- Seek feedback to expand clinical knowledge
- Consult with healthcare providers at all levels
- Negotiate with outside providers for services
- Maintain clinical practice privileges
- Maintain credentialing requirements
- Communicate fluently with stakeholders
- Facilitate information exchange and goal achievement
- Ensure adherence to laws and professional ethics
- Maintain current Basic Cardiac Life Support (BCLS) certification
- Comply with Safety, Health, and Environmental plans
- Comply with Quality Assurance plans
- Maintain regular and acceptable attendance
- Complete all required training
- Perform other assignments and duties as required
