- •Collaborate with healthcare providers to manage patient care
- •Escalate clinical findings to senior team members as needed
- •Complete status reports upon patient admission and during their stay
- •Visit or call providers for medical updates on remote cases
- •Liaise with providers and patients to gather detailed clinical information
- •Escalate concerns about suboptimal or harmful care to leadership
- •Assist with obtaining discharge summaries and hospital records
- •Ensure medical records are updated with accurate information
- •Provide on-site patient support services when feasible
- •Establish and maintain relationships with healthcare providers
