- Develop time-sensitive treatment plans for case-managed patients
- Assess patients via telephone using established protocols
- Establish priorities and monitor patient care progress
- Oversee discharge-planning activities for optimal timing
- Assist healthcare finders in identifying patient needs
- Negotiate and administer product/service agreements
- Recognize and report quality and risk management issues
- Use VA/DOD Clinical Practice Guidelines for standardized care
- Ensure optimal stewardship of healthcare resources
- Evaluate existing practices and outcomes using research tools
- Manage information systems for health and practice improvement
- Conduct data collection, tracking, and analysis
- Design and present computer-generated outcome analysis
- Understand pharmaceutical effects and complications
- Assist patients in optimizing function and self-care
- Educate patients on complex medical and psychosocial issues
- Seek feedback to expand clinical knowledge and performance
- Consult with healthcare providers at all levels
- Maintain clinical practice privileges and credentialing
- Communicate effectively with peers and stakeholders
- Coordinate and facilitate team communication and goal achievement
- Adhere to local, state, and federal laws and regulations
- Maintain current BCLS certification
- Comply with safety, health, and environmental policies
- Maintain regular and acceptable attendance
- Complete all required training
